Yeah. The quote can easily include the facility, doctor, nurses, anesthesia, labs, imaging, and anything else they reasonably expect. Just list everything that could be charged and let the patient decide what to do. Unexplained surprise bills months later shouldn't be normal.

Yes, and the quote needs some teeth. If the final bill runs more than like 10% over without patient approval, the hospital should eat the difference.
